What it is: The Basic Food Program helps low-income individuals and families obtain a more nutritious diet by supplementing their income with Basic Food benefits.
How to obtain a Help Warehouse Food Card

A punch card is needed to receive foods distributed through Help Warehouse.

1. HEAD of HOUSEHOLD -- Bring on of the following:
|
| a. Current utility bill with address and ID or |
| b. Rent receipt with current address and ID |
| (ID) must show same address as bill) |
| 2. OTHER ADULTS in HOUSEHOLD (18 years or older): |
| a. ID with same address as head of household |
| ACCEPTABLE ID INCLUDES (bring one of the following): |
| A. Drivers license |
| B. Checkbook |
| C. Medical Cards |
| D. Voters Registration |
| 3. PROFF of CHILDREN (under 18): |
| a. Birth Certificate |
| b. Medical Cards |
| c. Report Card |
